Extract or reconsider memory identities

Use the target server's normal authentication and an explicit exact Space ID. No cross-Space fallback is performed. Requires current write and model authority; permitted agents may request machine extraction. Uses the configured model and applicable model billing/admission. Omit mention_ids (or use null) with no new_spans for initial extraction only when there are no existing occurrences. An eligible interactive request invokes the model, including a repeated request after an empty result; background initial-pass deduplication is separate. When occurrences already exist, use selected mode. An explicit mention_ids array selects machine occurrences, optionally with new_spans. Human-confirmed choices are preserved. This is a bounded request for one Memory, not whole-Space replay or automatic profile creation.
